Blue cheese of any kind: I don't care whether it's gorgonzola, stilton, roquefort, or whatever; I hate, hate, hate the moldy taste.
Shrimp: Something about the texture weirds me out. It's kind of rubbery, and I can't get past the feeling of biting into the skin.
Bananas: I hate the mushy/slimy texture and the smell. The taste actually isn't so bad if I could get it sans those other things.
Cooked salmon: I like raw salmon and often get it at sushi restaurants, but I hate the way the flavor changes when you cook the fish.
Raw carrots: I don't mind them diced or julienned in a salad, but I will not just sit down and eat raw baby carrots. Even my beloved ranch dressing cannot make them taste good. I do love cooked carrots, though, because I'm weird like that.
